Online Consultation
Existing
patients are now able to request online consultations with their
doctor. We know that coming in for an office visit is not always
convenient and has a real cost to you as a patient; co-payments, gas
and parking, lost wages, child care and other costs of office visits
add up and we are sensitive to these costs and value your time. Our
Online Consultation service, which has a modest associated fee, is
designed not only for your convenience, but also to save you money when
your questions, that previously required an office visit, can be
adequately answered online. Online Consultation may be the ideal
service for you; it can serve the needs of many and we are happy to
offer this new service and convenience to you.

Appointment Information
To
schedule an appointment you may either call (919) 934-3015 ext. 3 or go
to patient login, make a login and do an appointment request on-line.
If you are a new patient or have not been seen at our office in the
past 3 years please click on Patient Resources, scroll down to New
Patient Information and print off the Patient Registration/Financial
Policy (3 pages)and Health History Questionnaire (2 pages) to fill out
and bring in with you to your appointment. If you are being seen for
pregnancy, please also print and fill out the Genetic Questionnaire (2
pages)in addition to the previous forms.
